Growing Great Families Offers an 8 week family group, Growing Great Families, for West County persons of color who are at risk for HIV or Hepatitis C. HIV testing and Hepatitis screening provided. Dinner is provided by all family participants.

HIV/AIDS Case Management:  Provides direct Case Management and substance abuse counseling services for people living with HIV/AIDS in Contra Costa County.   Case Managers assist clients in maintaining a healthy lifestyle, tracking and attending medical appointments, obtaining housing, food, and transportation support.

Perinatal Outpatient Substance Abuse Program:  Provides a five-day a week intensive outpatient treatment program designed to support the successful recovery of pregnant women and/or mothers with young children. This program includes counseling,  transportation, childcare and comprehensive family support.

Substance Abuse Assessment and Treatment for Youth and Adults:  Provides substance abuse assessment and treatment programs to adults and youth with substance abuse problems.  Payment  is on a sliding scale.   New Connections offers a full range of treatment groups for the general population as well as those court mandated to treatment —Prop 36, PC1000 in English and Spanish), Drug Court (FADS) and Offender Treatment Program.

An Intensive Outpatient Treatment program for insured or privately paying adults who are addicted to drugs/alcohol is also available on Tues/Thurs evenings and Saturday mornings combining substance abuse treatment and mental health support.  Referral to outpatient detoxification is made to Gracer Medical Center in the San Ramon Valley.

Substance Abuse Prevention for Youth: Provides prevention education, life skills, mentoring,  and individual counseling  to youth ages 12-18  who are at risk of using/abusing drugs and/or alcohol.   “Project Success” is provided in school and community settings.

Workplace Drug Testing:  Provides drug testing for employer groups (pre-employment drug testing for new hires or random drug testing for current employees). Substance abuse assessments also available. Call for more information.

Drug Free Communities is creating a collaborative in the Monument Corridor to address the community’s ability to reduce youth access to drugs and alcohol, improve family knowledge of ways to prevent youth from drug use, and reduce sales and trafficking illicit drugs in the community.
